Premier Health is pleased to offer you access to your personal medical information via a secure Web-based version of our computer systems called MyChart.
We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.
Premier Health will make its best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff that needs to respond to an electronic inquiry or other communication may not be immediately available so a patient should allow at least two (2) business days for a response.
Accordingly, emergency situations requiring immediate attention should not be submitted electronically.
Furthermore, with respect to any electronic communications sent by the patient, we are only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services.
Once your account has been activated, Premier Health MyChart will become the primary vehicle through which you will receive important messages from your primary care physician and selected test results and other personal medical information. (You may continue to receive communications via the United States Postal Service or other means from Premier Health specialty physicians)
Through Premier Health MyChart you may request or schedule new appointments and prescription renewals 24 hours a day 7 days a week. You must provide us with your email address when activating your Premier Health MyChart account and keep your email address current via the Personal Preference link in MyChart. No confidential information will be sent to your email. but you will receive an email notification whenever a new result has been released to our Premier Health MyChart account. Furthermore, you may receive from time to time important email notices and messages from the MyChart team. You therefore MUST verify and update your email address via the personal preferences link to ensure that your receive notification of newly released information in a timely manner.
Never send any messages requiring urgent attention using Premier Health MyChart. Delays can occur based on volume and availability of staff, the complexity of your condition, or technology occurrences outside of Premier Health Partner's control.
Therefore, for all urgent medical matters, contact your physician office directly , go to an emergency room, or call 911.
For non-emergency situations, support or other questions, contact your physician's office during regular business hours.
In addition to all other Terms and Conditions of Use contained herein, if you upload any photograph or digital image to Premier Health MyChart, you hereby represent that, including without limitation, the photograph or digital image:
You must understand and acknowledge that any communication you receive in response to the photograph or digital image is based upon what your primary care physician is able see at the time it is viewed. Neither Premier Health nor any physician has any control over the clarity of how photographs or digital images are taken, uploaded, transmitted or ultimately appear on any computer or mobile device. In no event should you wait to seek immediate attention if you feel your condition is worsening or if it is different than what the physician is able to discern from the photograph or digital image. Moreover, any advice or recommendation you may receive from your physician based on submission of this photograph or digital image, is not a substitute for a physician examination and/or an office visit.
You further understand that Premier Health has the right to remove any photo or digital image that it determines in its sole and absolute discretion is inappropriate, offensive, in violation of these Terms and Conditions of Use, in violation of any state or federal law, and/or does not otherwise belong in Premier Health MyChart.
MyChart may offer links to related medical Web sites not managed by Premier Health. These Web site link(s) are for a patient's informational purposes only. Premier Health does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis.
Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 12 or older may not view without consent of the minor patient. Because of these requirements, we cannot offer the full spectrum of services to parents of minor patients in this age category. Thus, when a minor patient reaches age 12, restrictions will be placed on MyChart access until he/she reaches age 18.
Individuals age 18 or older may request proxy access to another individual's MyChart account by completing the Proxy Access portion of the MyChart Consent Form and submitting it to Premier Health. Such access will only be granted to parties with parental rights or legal guardianship over the MyChart account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to account holder's medical information. Only one person will be granted proxy access to a MyChart account. A MyChart account will be activated for both the proxy and the account holder. If the proxy's legal relationship with the account holder changes, the proxy must inform the clinic immediately. Premier Health reserves the right to revoke proxy access at any time for any reason.
Premier Health MyChart is designed as a secure internet-based environment through which you may receive certain confidential medical information about yourself released at the discretion of your physician. Once you sign up for Premier Health MyChart, you will be given a one-time access code that you will use to activate your account. You will then create your Premier Health MyChart ID, password, and password hint. Please Do Not Share your password with anyone. The individual Premier Health MyChart ID and password you choose when you first log-on to Premier Health MyChart are the keys to protecting the confidentiality of your information. You are responsible for remembering your Premier Health MyChart ID and password and for keeping them confidential. We strongly recommend that you write down your Premier Health MyChart ID and password and keep them in a safe place for future reference.
Patients requesting use of MyChart are required to have a valid up to date email address. Patients who are users of MyChart should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyChart. This means that any person with access to a patient's e-mail will be able to see this notification. This could include the patient's spouse, employer or anyone else that can access a patient's e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address.
Please know that if you send us an e-mail communication, it may be shared with the Premier Health staff that assists the physician in providing the patient's medical care. A patient's confidential medical information on MyChart will be accessible only to appropriate clinical staff.
We afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by Premier Health in any other medium. Premier Health is committed to protecting the confidentiality of your medical information. We limit Premier Health employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses MyChart. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.
For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Internet Explorer version 8,9,10 or 11, Firefox 33+, Chrome 38+ or Safari 6). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).
You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Premier Health by viewing the digital certificate. This certificate verifies the connection between the Premier Health server's public key and the server's identification.
User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyChart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.
All content included on Premier Health Web sites, including, but not limited to, text, photographs, graphics, button icons, images, artwork, names, logos, trademarks, service marks and data (the "Content"), in any form including the compilation thereof, are protected by U.S. and international copyright law and conventions. The Content includes both Content owned or controlled by The Premier Health and Content owned or controlled by third parties and licensed to Premier Health. Except as set forth below, direct or indirect reproduction of the Content, in whole or in part, by any means, is prohibited without the express written consent of Premier Health.
Premier Health grants a limited license to each user to make personal use only of the Web site and the associated services in accordance with these terms and conditions of use. This license expressly excludes, without limitation, any reproduction, duplication, sale, resale or other commercial use of the Web site and the associated services, making any derivative of the Web site or the associated services, the collection and use of user email addresses or other user information, including, without limitation, health information or any data extraction or data mining whatsoever. Through this License, you may be able to upload third party data into MyChart or download limited medical information from MyChart to select third party sites
